There are days when I receive over 50 of these spam comments but they don't make it live because the spam remover is pretty awesome. As for the username issue, the person got around it by using the same username but different email and web address. The only way to stop this is to make all who leave comments register on the site first but this would hamper interactivity so I will just have to monitor it closely instead.
